Outreach program brings healthcare to the patient

Health Plus General Practice’s outreach service, ROMS (Residential Outreach Medical Service), has been designed to bridge the gap between residential aged care and general practitioners (GP).

A collaborative service between GPs, nurses, pharmacists and the aged care facility, as well as the patient and their family, ROMS is about proactive, preventative and holistic healthcare.  

Health Plus GP, Dr Ming-Chien Li, says the service has greatly benefited patients, as well as their families and the facilities, who have appreciated Health Plus’ proactive approach to healthcare management and their ability to respond to requests within 24 hours.

Dr Li has been involved with the program since its inception, and is the Senior GP working alongside the ROMS team to treat patients at the request of the facility or their families. He conducts weekly visits and health assessments, coordinates treatments with pharmacy and nursing staff, and meets with patients and their families to discuss any and all aspects of their care.

“Not many GPs can do regular check-ups for their patients in a nursing home,” he says. “We’ll have a good conversation with the patient about any issues they’d like to raise and we look into everything all together. It’s full coverage of the person, instead of just one issue at a time.”

Dr Li also performs additional and afterhours visits, at the request of the patient or their carers, and regularly meets with families to gain a better understanding of the patient and their needs.

“If the patient cannot raise the issue or cannot fully understand what kind of concern they have, then we will meet with their family members so we can get the whole picture,” Dr Li says.

Minor procedures and vaccinations can also be performed on site, and ROMS is fully covered by Medicare. The program is an extension of Health Plus’ commitment to providing high quality and holistic care to every patient.
