Medical outreach service provides convenient, continual care

Health Plus General Practice in Casey is pleased to introduce their latest service offering: the Residential Outreach Medical Service (ROMS).

ROMS has been designed to bridge the gap between residential aged care and a patient’s preferred doctor. Led by the experienced and dedicated team of general practitioners (GP) at Health Plus, ROMS is a collaborative outreach service run in conjunction with nurses, pharmacists and residence staff to ensure continuity of care for residents of aged care facilities.

Dr Wen-Shan Sung from Health Plus says the program is about developing a new healthcare delivery format to service these vulnerable groups.

“We are servicing this gap in care that’s out there,” he says. “Very often a patient will end up in the hospital because the GP is not available when something happens.

“With ROMS, we can provide consistency of care that’s proactive rather than reactive,” he says. “Any other preventative health issues can be addressed before a significant event arises.”

Dr Sung says COVID-19 has seen a lot of older or elderly patients who would usually visit their GP avoid making the trip into the practice. He says the ROMS service has increased feelings of safety for these residents, who can now have care come to them within their aged care facility.

ROMS services include weekly scheduled on-site visits by a GP, with additional visits available on request, and annual comprehensive health assessments with a detailed health assessment report and action plan.

Those enrolled in the ROMS program also have access to a high-priority triage number and a GP after-hours on-call service until 10pm. Minor procedures and vaccinations can also be performed on site, and ROMS is fully covered by Medicare.

Health Plus was established in 2018 with a vision to provide a better healthcare experience for patients and their families. ROMS, alongside the practice’s other services, allows the Health Plus team to provide high quality and holistic care to every patient.
